End-of-life care involves many different providers. Here's how Passaggio fits into the picture:

Hospice Services

Hospice provides direct end-of-life care: medical visits, nursing support, medications, equipment, and 24/7 availability for patients in their final months.

How we help: We can help you understand when hospice is appropriate, what different hospice programs offer, how to choose the right provider, and how to have the conversation about transitioning to hospice care. We help you arrive at that decision with clarity and confidence.

Palliative Care

Palliative care teams provide ongoing symptom management and support for people with serious illness, often alongside curative treatment.

How we help: We can help you understand what palliative care offers, whether it's right for your situation, and how to request it from your medical team. We help you know what to ask for.

Your Medical Team

Your physicians, specialists, and nurses provide diagnosis, treatment, and ongoing medical care.

How we help: We help you understand what your medical team is telling you, prepare questions for your appointments, and communicate your goals and preferences clearly. We complement their care by ensuring you're an informed, empowered participant in your own healthcare.

Legal & Financial Advisors

Attorneys handle wills, trusts, and legal documents. Financial planners address estate and insurance matters.

How we help: We focus on the medical and healthcare decisions—advance directives, goals of care, treatment preferences. We help you clarify these decisions so you can communicate them clearly to your legal and financial advisors.

Faith & Spiritual Support

Clergy, chaplains, and spiritual advisors provide religious guidance, sacraments, prayer, and faith-based support during end-of-life journeys.

Our approach: We have great respect for the role of faith in end-of-life decisions. Many people draw profound strength and comfort from their religious traditions, and we honor that completely. We work within whatever spiritual framework is meaningful to you and your family—our role is to support your journey, never to interfere with your religious beliefs or practices.

Your Medical Team

Your physicians remain responsible for your medical care. We help you understand and communicate with them more effectively—but we don't replace them.

Your Nursing & Hospice Care

Medications, symptom management, and clinical care belong with your hospice or nursing providers. We help you navigate those relationships with confidence.

Your Caregivers & Home Support

Bedside presence, personal care, and daily living support are provided by your family, aides, or home health services. We help ensure everyone understands the plan.

Your Faith Community

Your clergy, chaplain, and spiritual support are sacred. We work alongside your faith traditions with deep respect—never in place of them.

Our Role in Your Journey

We are counselors and guides, not clinical care providers. Your medical team and hospice/palliative providers carry out the care — and they make decisions with you through a process called shared decision-making. Our job is to prepare you for that process: to ensure you arrive at those conversations informed, clear about your values, and ready to participate as an equal.

This focused role allows us to give you something your busy medical providers often cannot: dedicated time to talk through your situation, answer every question, and ensure you truly understand your options before you make decisions.
